I hereby authorize the work described above, and agree to pay for the requested service upon completion. I understand that Systems Plus Computers, Inc. will perform the requested work to the best of its ability, and that they are not liable for any damages caused by delay or other performance. The sole remedy for Systems Plus Computers, Inc. liability of any kind, including negligence, shall be limited to the re-performance of any defective service by Systems Plus Computers, Inc. and in no event will include any incidental or consequential damages.
I am responsible for having enough licensed copies of software for my equipment.
I understand that reformatting hard drives or reloading software may result in loss of my personal data, and I am responsible for making and maintaining reliable backups. The transfer, conversion, or recovery of data has the potential for data corruption or loss. Systems Plus Computers, Inc. cannot be held responsible for any loss of data during the repair of my system.
Equipment left beyond 30 calendar days is considered abandoned and will become property of SPCI.
Manufacturers’ warranties cover hardware malfunctions only. These warranties do not cover malware/virus removal, loss of data caused by a virus, OS installation or upgrade, data recovery, application installation, or other program and software related issues.
Only a certified technician can evaluate a computer or device for warranty coverage. If the technicians evaluate your computer or device as being covered under warranty, that warranty will cover all parts and labor costs. This would include the Service fee and any labor to install parts. This would NOT include data, as data is not covered by any warranty.
By submitting this form, we are accepting it as your signature, agreement, and authorization for this transaction.
If my system is not covered by a warranty, or any damage is found that is not covered by warranty, there will be a $200 fee charged to cover Service and/or damage assessment which is due at in-store check-in. Any repair costs above and beyond the initial diagnostic will NOT be done without prior customer approval.
Information specific for Apple Devices:
- "Find My.." must be turned off in order to be serviced.
- Apple Policy is any Apple devices repaired under warranty will need to stay at Systems Plus for the entire duration of the warranty repair.
